Not always. Mastectomy, for example, can be performed without prior hormone therapy. Metoidioplasty does require it, because the result depends on clitoral growth with testosterone. In your assessment we explain what applies to your case.
It depends on your priorities. Metoidioplasty is less invasive and preserves sensation and natural erections, but the neophallus is smaller. Phalloplasty allows a larger phallus and penetration with a prosthesis, in exchange for more surgical stages and donor-site scars. In consultation we help you decide without pressure.
A mastectomy is resolved in a single operation with a recovery of a few weeks. Complete genital surgery may require one to three stages over 12-18 months. We design a schedule adapted to your life.
